Plaque & Tartar Evaluation

Getting Started with Sage Dental Specialists
If you’re visiting us for the first time, welcome! To make the first visit as smooth as possible, new patients are encouraged to complete our New Patient Registration Form ahead of time. This helps the team get to know your medical history and prepare for your appointment.
If you’d prefer to take care of the paperwork at the clinic, that’s fine too! Just plan to arrive about 10–15 minutes early to ensure there’s plenty of time to finish the forms comfortably.
